Need to Update Health Records?

 Updated records may be submitted through:

• email records to nurse 
• sent in with student and dropped off in the nurse's office
• dropped off at the school during school hours.

Communication

It is the responsibility of the parent/guardian to tell the school nurse about any health concerns or chronic health issues that your student may have. Please be sure to complete the returning student registration form each year for your student. If your student's medical status changes during the school year, please contact the nurse.

Emergency Action Plans

Per school district policy, if your child has asthma, severe allergies, diabetes, or a seizure disorder, an emergency action plan is required to be on file with the school nurse and updated each year. EAPs are completed by the student's doctor. Emergency medications must be provided to the school as ordered by the doctor.

Medication at School

When possible, students should take medication at home before coming to school.  If your student needs to take medication while at school, a parent or guardian must complete the online LPS Medication Authorization form and return it to the nurse with the student's medication.  
All medications* must be kept in the nurse's office.  Medications will be administered in accordance to nursing guidelines and Liberty Public Schools medication administration policy.
*Students with diabetes or asthma may carry emergency medication with the completed self- administration documentation.

What is the Acetaminophen Policy?

Regular strength acetaminophen (generic Tylenol) is stocked and available to LPS students starting in 6th grade who have signed acetaminophen permission on file with the school nurse.  Parents will be notified if a student frequently requests acetaminophen.

• Free from vomiting for 24 hours after the LAST time I vomited.

• Diarrhea in the last 24 hours

I am ready to go back to school when I am...

• Free from diarrhea for the 24 hours after the LAST time I had diarrhea.

What injuries require a doctor's note?

• Any injury which requires special restrictions for PE class
• Any injury that requires a student to use crutches
• Concussions that require special accommodations in the classroom such as delayed testing, reduced screen time, etc.

What if my student is on crutches?

• If your student is on crutches please bring them to the nurse's office on their first day back to school. 
• Your student will be given a "buddy pass" which allows for early passing time with another student to carry your student's belongings.
   

Incoming 8th graders must have record of Tdap and meningococcal immunizations on file with the nurse.

Missouri State Law requires that children must have up to date vaccination records on file at school. It is your responsibility as the parent/guardian of your child to give the school updated official records of immunization.  If your child is missing the required vaccinations for school, your child WILL NOT be able to come to school until the needed vaccines have been given and the written proof is given to the School Nurse.

There are 2 reasons that the state will allow your student to attend school without the required vaccines:

• Medical Exemption - there is a medical reason your child cannot receive a vaccine. You will need to get the exemption card from your child's doctor.
• Religious Exemption - it is your religious belief that your child should not get a vaccine. You may now access the Religious Exemption card directly from Missouri Department Health and Senior Services website.  The card must be completed in full by a parent or legal guardian and turned in directly to the school nurse.

The Missouri Department Health and Senior Services provides a complete listing of the immunization requirements for school age children.

What Health Screenings Take Place?

Height and Weight will be done in conjunction with PE teacher and curriculum.

Dietary Restrictions

If your student requires special diet restrictions due to severe allergy, intolerance or other medical condition, a Medical Statement completed and signed by the child's doctor must be on file with the school nurse and Nutrition Services. 

Discontinuation of Diet Restrictions

In the event you wish to discontinue a previously registered restriction, a Discontinuation of Diet form must be signed by both parent and physician and submitted to the school nurse.
